Staff did not have clear guidance on handling choking risks or giving spare medicines for constipation that went on for days.

Where people were at risk of choking, risk assessments did not include guidance for staff on how to support people during choking episodes that was individual to them. One person went 6 days without opening their bowels.

Enough staff to get by but some agency workers cover shifts while hiring more permanent ones.

Agency staff were still being used while recruitment was being carried out.

Quality checks missed problems with risk assessments and medication until the inspection.

Quality assurance systems were in place, however, not effective in picking up areas of concern we found on inspection. Medicine audits had been carried out, however, did not identify that PRN medicines were not being managed effectively.
