Staff did not always assess risks to people's health and safety or mitigate them where identified. Medicines were not managed safely.
We received mixed feedback from staff about how well they felt supported. Comments included, “Registered manager listens to us and is trying to get things done but there’s just not enough staff for the people we’ve got”
There were insufficient staff to meet people’s needs and keep them safe. People, relatives and staff raised concerns about the staffing levels, particularly at nights and weekends.
Staff did not have the training to understand this condition. Records showed 3 fire drills had been carried out since the beginning of 2024; however, no night staff had participated.
There was a lack of effective oversight and management of the service which had impacted on the quality of people's care.
The provider employed an activity organiser who people and relatives said was very good. However, this staff member also had other roles which impacted on activity provision. This meant planned activities did not always take place.
Not enough staff especially nights and weekends, so you get stretched thin and people go unsupervised.
At night there were 2 staff on duty for 23 people accommodated over 2 floors. People, relatives and staff raised concerns about the staffing levels, particularly at nights and weekends.
Unsafe medication practices and unassessed risks like bed rails and heaters put people in danger.
Medicines were not managed safely. No risk assessments were in place for people who had bed rails. There was no risk assessment in place for another person who had a free-standing oil heater.
Poor leadership oversight means safety issues and staffing shortages are not fixed quickly.
There was a significant lack of oversight at the service and quality assurance systems and processes were ineffective.
