Medication errors happen because timings with food are not recorded and doctor's instructions are missed.

People did not always receive their medicines as prescribed. For example, some people were receiving medicines with other medicines when the prescriber's instructions advised not to do this. Staff did not record the time medicines were given.

Staff skills are not reliable enough after training, with repeated equipment accidents showing gaps.

Although staff had been trained in moving and handling, we could not be assured of their skills and competence as there was continued incidents involving equipment.

Managers' checks and audits miss key problems like poor care plans and risks.

Governance systems had not always identified the areas we found at this inspection. For example, the audits had not identified the concerns we found with medicines or care plans.
