"Staff feel well supported in a positive culture with enough staff available, but big safety problems like unsafe medicine handling and gaps in training put people at risk."
The service was in breach of legal regulations in relation to safe care and treatment, safeguarding and safe environments... The provider failed to ensure staff had access to people’s full care plans and risk assessments... unsafe medicine management.
Staff told us they felt well supported by the provider... Staff told us it was a positive place to work... However, staff told us they had limited opportunities to meet as a team to give feedback... staff meetings had not taken place for a period of time.
We observed there were sufficient staff available to support people... However, the provider had not always kept complete records of the checks they completed for staff recruitment... no evidence of a dependency tool to assess the number of staff required.
Not all staff had completed training in safeguarding... Staff had not all received training with regards to people’s specific health needs. For example... 5 staff providing care had not completed epilepsy training... Staff had not all received health and safety training, or their training had expired.
The provider, who was also the registered manager, did not demonstrate they had the required leadership and management skills and up to date knowledge to ensure... risks were well managed. There was a lack of depth in the structure of the management team... no deputy manager... for approximately 3 years.
We received mixed feedback about the activities... One staff member told us people hadn’t been out since an activities coordinator left. The provider told us another activities coordinator had recently been employed, and we saw some people joining in with seated games.
Big safety problems like unsafe medication handling, missing legal papers for restrictions on residents movements, and fire risks in the building put people at risk of harm.
The service was in breach of legal regulations in relation to safe care and treatment, safeguarding and safe environments... provider had not ensured all people who were subject to restrictions... had the appropriate legal authorisation... failed to act on all fire safety recommendations.
Lots of gaps in staff training including no epilepsy training for some caring for residents with seizures, missing infection control training, and poor skills on the new records system.
Staff had not all received training with regards to people’s specific health needs. For example, some people had seizures, however, 5 staff providing care had not completed epilepsy training... Not all staff had completed training in safeguarding... Staff had not all completed appropriate infection prevention and control (IPC) training.
Weak leadership with no deputy manager for 3 years, missing policies and records, and incomplete staff recruitment checks make oversight unreliable.
staff told us there had not been a deputy manager at the service for approximately 3 years... The provider was unable to provide CQC with a significant number of documents... shortfalls in the provider’s governance processes around staff recruitment... no evidence all staff had a current DBS in place.
