Staff did not always assess risks to people's health and safety or mitigate them where identified. People did not always have care plans to guide staff with safe practice.
Staff did not always feel they could speak up and that their voice would be heard. Some staff shared with us they did not feel listened to when they raised concerns.
Staff informed us they felt they needed an extra member of staff to provide support to people. Staff said, “Some days are worse than others, we could do with another member of staff.” and “We could do with more staff in the mornings.”
Most staff had up to date training however we found practical moving and handling training had not been provided for 18 months. This meant staff training in relation to supporting people with their mobility and transfer might not have been up to date and current.
The registered manager did not have a system in place to review trends and themes in the service and use this as a learning opportunity to improve safety at the service.
There continued to be a lack of meaningful activities for people to be involved in to meet their social and wellbeing needs. Opportunities for activities and social engagement were limited with an over reliance on the television being on to entertain people.

A member of staff told us there was a lack of direction given to them by the management and they felt when issues were raised, they were not resolved.
Staff feel stretched on busy days and say they need more team members.

Infection prevention and control practices were poor, and the environment was not always maintained to an adequate standard and was unhygienic with equipment not cleaned and inappropriately stored.
