The service was in breach of legal regulation in relation to managing medicines, premises and equipment, managing risks posed to people, staffing and infection prevention and control.
Staff told us the morale within the team was good and they worked well together. A staff member said, “I love it here, it is a really good atmosphere, and we all pull together.”
At times in the afternoon and at night, only one member of staff supported ten people. Some staff told us they felt there were not always enough staff.
Staff were trained in a range of subjects to enable them to carry out their roles effectively. Staff received regular supervisions and were supported with their professional development.
Leaders did not have the skills, knowledge, experience and credibility to lead effectively. Daily assurance checks were not undertaken by the management team.
An activities coordinator had recently been introduced, with activities such as baking, arts and crafts, shopping and meals out. However, activities and stimulation were not consistent. We observed long periods where people were not engaged.

Medicines were not always safely managed... some areas were found to be visibly dirty. For example, we found some soiled flooring and soiled chairs. Out-of-date hand gels were in use.
Managers miss key checks and don't spot problems like safety gaps, so leadership doesn't feel strong or reliable.
Daily assurance checks were not undertaken by the management team and concerns we found were not identified or actioned... quality audits did not identify or address concerns with medicines, infection control, environmental safety, staffing and records.
