Accidents and possible abuse weren't always checked out properly or told to the manager, so risks weren't fixed.

Incidents were not always reviewed to determine if there was possible abuse which needed further investigation. We raised these incidents with the registered manager who was not aware of them

Staff culture problems like disrespect towards residents and staff not engaging properly weren't sorted quickly.

Staff did not always demonstrate a respectful approach towards people. For example, we had concerns about how some staff spoke or had written about people. The registered manager had identified some issues in relation to staff culture, but these had not been quickly addressed.
