"Improved staffing with stable teams and a positive culture where staff feel listened to, but safety risks are not fully managed and monitoring lacks thoroughness."
Not enough improvement had been made at this inspection and the provider was still in breach of [Regulation 12]... individual risks were not always highlighted... incidents were not always recorded to enable monitoring.
Staff culture had improved, no staff reported concerns of bullying as they had at the previous 2 inspections. Staff said they felt listened to and were able to speak up if they needed to.
Staffing levels had improved, and safe staff recruitment practices continued to be in place. The levels of agency staff had reduced and the agency staff supporting people now were regular agency staff who were treated as part of the team.
Staff continued to complete their training and the provider had a system to check this. Staff were supported in their role through regular supervision meetings with their line manager.
The provider had introduced new monitoring systems since the last inspection however; these were not always robust and required further improvement to make sure people received safe and good quality care.
Managers are not fully spotting issues like poor risk records because their monitoring and audits are not thorough enough.
The provider continued to fail to operate a robust quality assurance process to understand and have oversight of the quality of the service and ensure any shortfalls were addressed. This is a continued breach of Regulation 17.
Risks like choking and diabetes complications are not always fully managed or recorded properly, leaving some safety gaps.
The provider continued to fail to assess the risks to the health and safety of people or do all that was reasonably practicable to mitigate risks. This is a continued breach of regulation 12.
Only standard training to keep up to date, no specialist courses or funded qualifications mentioned.
Staff had the training they needed to provide people's care... The provider made sure staff kept their training up to date and had a system to keep a check on this.
