Risks had not always been fully assessed to keep people safe and protected... records of restraint incidents needed improvement to show that less restrictive strategies had been considered prior to the use of restraint.
Staff understood and felt confident in their role. They told us they felt well supported and that they could talk to management at any time, feeling confident any concerns would be acted on promptly.

Bosses' checks missed problems like poor risk records and restraint logs, so they only fix things after inspectors point them out.
The provider did not have effective systems and processes to assess, monitor and improve the safety of the service. This was a breach of Regulation 17.
