Managers audits missed problems with care records and medications, so oversight is not strong enough to catch issues early.

Audits and checks had not identified the concerns we found during this inspection around care records, staff interventions and medicines.

Gaps in medication records and care logs mean risks like pressure sores or distress are not always handled properly.

People's medicine records did not always reflect they received these as prescribed... records for 3 people who required support to reposition... evidenced gaps of 8 to 12 hours or more.

Only basic activities like games and outings with no dedicated coordinator, so the job is mostly routine personal care.

There was a schedule of activities available for people to join in with, and these were advertised around the service. People told us they picked activities that interested them. For example... carpet bowls.
