High staff vacancies mean heavy use of agency workers and sometimes not enough people on shift for outings or events.

There were still a high number of staff vacancies, the provider used regular agency staff... One person told us, 'Sometimes I can't go out because there aren't enough staff.'

Risk assessments are not always updated straight after incidents happen.

Risk assessments had not always been reviewed and updated following incidents. For example, one person... had several incidents... the risk assessment had not been updated.

Only standard training like safeguarding and medicines, no mention of specialist courses or funded qualifications.

Staff had training on how to recognise and report abuse... Staff had completed infection prevention and control training... competency records were in place... for medicines.
