Managers and provider missed big problems like incomplete safety records and care plans because their checks were not thorough enough.

Health and safety checks were carried out but were not always recorded or recorded in detail. For example, fire door checks were not documented.

Training records were not up to date, so its hard to be sure there are no gaps in staff skills.

The provider's training matrix was not up to date at the time of inspection to give assurances about any gaps in training and how these were being monitored and addressed.
