We observed very little in terms of activity provision... games, quizzes and entertainments were provided... a dog comes once a week

Risk records have gaps like missing plans for limb contractures or wrong repositioning charts, so new staff might not give fully safe care.

one person living with severe contractures of the limbs had no plan in place... the person's repositioning chart did not reflect that their position was being changed in line with their care plan

Staffing is enough to get by with prompt responses, but nothing special like low agency use or great retention mentioned.

There were sufficient staff to meet people's needs... 'You could always do with more, but it's very good'
