"Activities support complex needs decently, but no permanent manager, poor staffing planning, and repeated medicines errors leave staff unsupported and safety weak."
there had been numerous medicines errors. The previous 3 inspections had identified shortfalls in the management of medicines, and this continued at this inspection.
Some staff told us they did not always feel supported especially at night or the weekend when the management team were not working. Staff told us they had not had regular supervision to discuss their training needs and practice.
There was no system in operation to determine the number of staff and range of skills required to meet people’s needs and keep them safe.
Staff had not received supervision every 3 months in accordance with the provider’s policy, this included nurses where only 1 nurse had received a clinical supervision. Although staff competencies had been completed, these had not always been reviewed nor was there evidence staff competency had been effectively assessed.
At the time of our inspection there was no registered manager in post.
Relatives told us staff had not always encouraged people to take part in activities, a relative told us, “They are good with looking after their complex needs and do it well. Where they do less well in the social aspect.”
No permanent manager in charge, so leadership is weak and staff lack oversight on shifts.
At the time of our inspection there was no registered manager in post. The current manager had been in post since February 2025, they told us they had started their registration with CQC but had not fully understood the process which had caused a delay.
No proper system to plan staffing levels or skills needed, leaving staff stretched without enough nurses or clinical support.
The manager was unclear about how many nurses were required on each shift. We reviewed the duty rota for the week of our onsite inspection, the number of nurses ranged between 1 and 4 on duty.
Lots of medication errors and missing risk guidance, so staff worry about keeping people safe.
