The provider had failed to ensure the ongoing maintenance of the environment which put people at increased risk of infection. Care planning was not always clear and consistent in identifying risk.

Per 2019 comprehensive inspection: Staff had access to training, supervision and appraisal. Training records showed a high completion of training courses, both mandatory and specific clinical courses for nurses.
