"Enough staff on duty, supportive team atmosphere and varied activities, but weak governance fails to spot risks independently and safety issues persist."
Systems had not been fully established to monitor and mitigate risks to the health, safety and welfare of people using the service. This placed people at potential risk of harm.

Systems had not been fully established to monitor and mitigate risks to the health, safety and welfare of people using the service. This placed people at potential risk of harm.
Training lacks a proper plan for refreshers or matching courses to people's specific needs and staff roles.
There was no system in place to alert management when refresher training was due, to support effective planning... no specific plan to monitor the range of training, in line with all the people's specific needs, staff's additional responsibilities or known areas of development.
