Not enough staff on duty and high agency use leaves everyone stretched and care rushed.

Staff, people and relatives told us they did not feel there were enough staff on duty to meet people’s needs. The provider used a high level of agency staff.

Medication records have gaps and errors like missing signatures and wrong timing.

Medicines administered covertly did not have all the required documentation... Prescribed creams were not always signed for. One person’s pain patch was prescribed to be applied once per week but was signed for as applied on the 29 and 30 August 2025.

Frequent manager changes mean poor oversight and slow fixes to problems.

There had been several recent changes with managers at the service which had resulted in ineffective oversight of the service, and implementing effective audit processes and driving improvements.
