We raised 2 safeguarding referrals to the local authority team in relation to the transfers of 2 people from the ground to the first floor and vice versa, due to our significant concerns for their safety.
Staff surveys indicated issues with the quality and amount of staff supervisions and the training opportunities offered.
We reviewed 3 new staff recruitment records and found the pre-employment checks for all 3 were not carried out in line with requirements.
We were not assured that the training provided to staff was sufficient or at an appropriate level to support staff with the correct knowledge, skills and understanding to effectively carry out their role.
