"Strong training, high staff retention and supportive management, but poor risk assessments for choking and pressure sores let it down."
Risks to people's health were not always assessed... conflicting advice on dietary consistency for choking risk... pressure damage care plans inadequate with gaps over 23 hours... Lessons were not always learned from incidents... breach of regulation 12.
Staff told us they felt well supported by the registered manager... Comments included, 'I can approach them anytime and they would listen, I do love it here.' A staff member said, 'The best thing about working here is looking after the residents here, it can be really rewarding...'
There was a high retention of staff who had worked at the service for many years under the previous provider. Staff told us there were enough staff... 'Permanent staff have been increased recently which has been brilliant.' Where there were shortfalls in the rota, these were filled by agency care or nursing staff.
Staff spoke highly of the learning opportunities... 'The training here is brilliant... interactive dementia training recently...' Records confirmed a high percentage of staff had completed their mandatory training and additional training... Staff received supervisions.
The registered manager was highly regarded by people's relatives and staff. They told us they were able to approach the registered manager with suggestions and felt listened to. Comments included, '[Registered manager]... are amazing. Very amenable. They both get out and about, they engage with the teams and the residents.'
The activity schedule... contained a mixture of social, religious and physical activities. We observed a ball game... a show with an external entertainer... An additional activity worker was employed following feedback... art club, movie night.
Risk plans for choking and pressure sores are often missing or wrong in records, so staff have to rely on word of mouth and mistakes can happen.
SaLT advice had not been clearly updated... One person... 'liquidised' consistency; their care records advised... 'pureed'... People who were at risk of developing pressure damage... did not have appropriate care plans... gaps of over 23 hours between position changes.
Quality checks and audits don't spot problems like bad care records, so issues build up without fixes.
Quality assurance processes were not routinely documented... Audits did not always identify concerns found at this inspection, such as, conflicting care plans... Systems did not identify these omissions.
Activities are mostly basic group games, entertainers and art, with no specialist creative programmes to vary the day.
People were encouraged to join in with organised activities. We observed a ball game... entertainer... The activity schedule... mixture of social, religious and physical activities... weekly art club... movie night.
