Staff training has big gaps, like only 20% done dementia and nutrition training due to COVID.

Not all staff had up to date training, the training matrix evidenced only 20% of care staff had received dementia training, and nutrition training.

Staff say there aren't enough of them, so call bells go unanswered for over 10 minutes and work feels very rushed.

Not all staff we spoke to felt there were enough staff on duty... call bells were not answered in a timely manner... over 10 minutes on three separate occasions.

Gaps in medication charts, risk checks for pressure sores and fluids, and injury records make safety unreliable.

Repositioning charts did not detail how often people should be repositioned... gaps in the recording... Medicine Administration Records (MAR) required improvement... Unexplained bruises and injuries had not always been recorded on body maps.
