Staff at the home had obtained funding from the Clinical Commissioning Group to set up an end of life tool called 'Doing it my Way'. This has been recognised at a number of awards including being a regional finalist in 'The Great British Care Awards' and were highly commended at the Royal College of Nursing awards.
