Poor management of medicines placed people at risk of not receiving their prescribed medication correctly. We made several referrals to the local authority safeguarding team following this assessment.
The providers values were not upheld by the management team and staff morale was low. Staff told us they did not feel supported within their role. Comments from staff included, 'I raised things, but nothing gets done about it'

We identified that out of 40 staff only 16 had completed fire safety training. Only 1 member of the night staff had completed fire safety training. The provider’s training matrix showed only 3 staff had completed infection control training in 2024 and only 2 staff in 2025.
The manager did not demonstrate an understanding of their responsibilities in providing safe and effective care. The manager did not recognise the inconsistencies we identified in how risks to people were being managed.
The service provided activities to meet people's needs, however this was only in a morning. During the afternoon people sat in communal areas with a lack of stimulation and little staff presence.
