Staff miss key training on moving people safely, first aid, and safeguarding, and some failed competency checks with no follow-up.

Staff had not completed mandatory training and refresher courses to perform their roles effectively such as with moving & handling, safeguarding and first aid... some staff had not achieved an expected pass mark when their competency was checked... no further training had been booked.

Managers lack oversight on staff training, so gaps in skills are not spotted or fixed quickly.
