Risk plans and care records don't always match what residents need right now, and there were mistakes signing off medications.

Care plans and risk assessments did not always reflect people’s current needs... the medicine administration record (MAR) recorded this had been administered each day.

Shift rotas aren't kept properly, and some didn't have the right staff gender mix or numbers for resident choices.

The provider was not retaining rotas... rotas shared were not always in accordance with people’s needs or choices. For example, 1 person requested a specific gender of staff to support with personal care on set days. However, there were days when the rota displayed an absence of this gender of staff.

Policies are outdated and audits missed problems like poor risk plans and water temperature issues.

Policies and procedures were not kept updated with changing legislation... Governance systems and audits were not effective when identifying or addressing the discrepancies found during this assessment.
