Heavy agency staff use, especially in dementia unit where 3 out of 4 were agency and did not know care plans well.

On the first day of our inspection 3 out of 4 care staff in the Hilltops areas were agency care staff. Agency care staff had not received the physical intervention training as stated in people's care plans

Gaps in training for physical support techniques when residents resist care, and not enough trained staff on shifts.

There were not always enough staff on duty trained in these techniques. Staff were inconsistent when demonstrating physical intervention techniques to the inspection team.

Medication errors happen because not always given as prescribed, and care plans lack clear risk guidance.

Medicines were not always administered in line with prescribing instructions. Care plans did not always detail physical intervention techniques adequately to guide staff to support people safely.
