Medicines were used regularly for minimising people’s distress without recording appropriate processes to ensure they were used proportionately and as the last resort. Risk assessments were not carried out when risk had been identified and actions were not always taken following incidents to mitigate or minimise risk of reoccurrence.
Staff who had worked in the service for a while, told us conditions were improving daily and the registered manager always checked on their wellbeing to ensure they were ok.
The recruitment process was not robust enough to ensure positions were offered to suitable applicants. We checked 3 staff files which all had gaps in their employment history with no explanations.
Although training relevant to their role had been completed by most staff, we found inductions for new starters were not always completed which meant we could not be assured new staff had been prepared to provide safe, compassionate and high-quality care.
Governance systems were not effective and did not record actions taken to improve the safety or quality of the service.
There were multiple activity coordinators who supported people to participate in group activities and provided one to one time for people who were either cared for in bed or chose to remain in their bedroom. Relatives told us there were some opportunities for people to access their local community such as visiting shops or cafes.
