roles and responsibilities within the management team were unclear and the providers’ approach continued to be reactive and not pro-active in driving sustained improvement. They did not have an overall development plan

We noted on all 3 visits to the service that people were not occupied with any meaningful occupation, which would help to reduce agitation and frustration
