The assessment took place due to concerns we received regarding poor culture in the service... There had been previous concerns regarding staff working cohesively together.
There was no evidence of appraisals having taken place for some staff within the last 3 years. Supervisions were due to take place on a regular basis, however some staff had not had a supervision for an extended period of time.
Staff had not received appraisals and supervisions as required. The provider did not complete Oliver McGowan training in relation to people with a learning disability or autistic people.
The provider’s monitoring systems had not always been effective as they had failed to identify and address the concerns we found during our inspection. Audits completed had not identified the concerns we found with safeguarding concerns, appraisal and supervisions.
