Medication records have gaps like missing stock checks and witness signatures, so safety processes are not fully robust.

we identified that when the administering process was carried out by two staff, the staff who did not witness the person taking the medicine signed the records... staff did not always record the checks of quantities of medicines... lack of 'as required' written protocols (PRN)

Only standard training and regular supervision with no specialist courses or funded qualifications mentioned.

Staff had ongoing, relevant to their roles training and they were regularly supervised.
