Safety practices have gaps like missing risk assessments for bed rails, skipped repositioning checks, and unsafe medicine storage.

We identified two people who had bedrails on their beds which had not been risk assessed or documented in their care plan... gaps of significantly more than 2 hours... Topical medicines were not always being stored securely... some gaps in records for the administration of people’s topical medicines.

Staffing is enough most times but mealtime deployment leaves people waiting, and recruitment records have gaps.

the deployment of staff at mealtimes was in need of review... the records, some of which they had inherited from the previous provider were incomplete.

Training covers basics and staff like the opportunities but competency checks have gaps for some nurses.

Competency checks were completed... but some gaps were identified for some nursing staff and other senior staff.
