People were at risk of harm relating to skincare, choking, malnutrition, falls and managing people’s behaviours. Care plans lacked clear guidance for staff. Medicines were not managed safely.
Whilst staff told us they felt better supported under the new provider, we found the culture of the service was not positive. We observed care that people received was task focused and not personalised.
People were at risk as there were not enough knowledgeable and skilled staff to care for people safely. We observed multiple occasions when there were no staff supervising people in communal areas.
The provider had a training programme. However, observation of staff practice did not assure us staff had learnt from their training and had the skills and knowledge to keep people safe.
The provider and manager did not have the skills, knowledge and understanding they needed to deliver a service that provided safe care and treatment to people living at the service.
Per 2022 comprehensive inspection: There was no formal approach for the staff team to engage people in activities. We observed staff support people on a one to one basis.
Not enough staff on duty, leaving residents unsupervised at times and causing rushing with no time to chat.
Most people told us there were not enough staff available to meet their needs or spend time with them. Comments included, “There’s not enough staff, they need more. They are always rushing with very little time to chat.”
Manager and provider lack the skills and oversight to identify risks or drive improvements.
The provider’s systems, processes and oversight to ensure risks were managed safely, were either not in place or not effective at identifying concerns and driving improvement.
Unsafe practices like poor medication handling, infection control failures, and not reporting abuse put residents at serious risk.
