Prescribed creams were still not being dated once opened... monitoring charts for bowel movements, repositioning, food and fluid intake and personal hygiene were not fully completed... The service was in breach of legal regulation in relation to reg 12 safe care and treatment.

Staff told us they were concerned with the number of changes in leadership and poor communication which they found demoralising. Some staff told us that when they had previously raised issues, nothing was done.

Staff told us they felt they were short staffed at times, and this prevented them to support people to go out when they wanted to... A lot of the staff are agency, and care is not very good.”

We identified some staff still had gaps in their training... Care staff had not completed falls training; this was booked during this assessment. Staff had not been trained in areas such as falls and positive behaviour support

there was no registered manager in post. Whilst recruitment was underway, interim support was being provided by a registered manager from a local sister service alongside the deputy manager

There were no daily activities despite residents requesting more during meetings. Whilst some people engaged in activities with the therapy team, these were not provided daily... no activity coordinator in place.
