No permanent manager in place, and recent management changes left staff distressed from past financial issues.

There was no current registered manager at the service but arrangements to address this were taking place. Some of the staff team told us they had been distressed by the departure of key members of staff last year due to financial irregularities.

Serious medication safety problems like unlocked drug cabinet and missing instructions for as-needed pain relief meds.

Medicines optimisation Score:1 ... ‘as required’ medicines protocols not always in place... The controlled drugs cabinet was not secured as legally required.

No activities coordinator right now, so the role is mostly routine personal care tasks with little variety or creativity.

People were not fully supported to experience a wide range of activities... as there was currently no activity co-ordinator.
