The audits and checks completed did not ensure that appropriate standards of quality and safety were always maintained. Some audits were not always completed as often as they should have been and where shortfalls had been identified, the necessary improvements had not always been made

Hard to keep everyone safe because low staff numbers mean you cannot watch communal areas properly.
