Various concerns around the safety of people’s care, including health and safety, infection prevention and control (IPC) and medicine concerns were found during the inspection.
Staff spoke positively about the support they received from the registered manager, and they told us they felt supported and valued in their role. Their comments included, “[Registered manager] is very approachable and really helpful”
Systems to monitor staffing levels were not robust. The provider did not have systems in place or use a dependency tool to determine safe staffing levels at the service. A staff member commented, “Sometimes there is not enough staff which is an issue.”
Staff completed an induction when they first started, and there were opportunities for supervision [one-to-one support sessions with their line manager].
Governance systems were not robust. Some of the health and safety and IPC concerns were found via the provider’s auditing systems in August 2025 and September 2025, however, timely action had not been taken to address the concerns and issues remained during our inspection.
An activity co-ordinator was employed 3 times a week and people told us they enjoyed engaging in the activities planned by the activities co-ordinator. However, during the remaining days of the week, there were limited and no meaningful activities for people to engage in.
No tool or system to figure out safe staffing numbers, so shifts get stretched at busy times and staff say there is not always enough help.

Serious safety risks like unsafe ways of moving people, poor cleaning and infection control, and mistakes giving medicines that were not fixed.
Moving and handling practices were unsafe. We observed a number of incidents of staff providing unsafe movement and handling of people... Quality and safety in administering medicines were not maintained, medicine monitoring systems were not robust... We reviewed several records related to cleaning and maintenance of people’s rooms and equipment and found vast shortfalls.
Weak oversight from leaders means known problems from audits are not sorted out quickly, leaving risks unaddressed.
