People were not always protected from the risk of potential health infections, and there were lapses in good food hygiene practices by staff... This was a breach of regulation 12.

Poor oversight let safety and maintenance problems like loose handrails and broken toilet seats go unnoticed.
Environmental safety concerns were not always identified... a safety handrail was not securely attached... a person's toilet seat was broken... provider's governance processes had not identified the safety issues.
Only enough staff to get by with no details on low agency use or super stable teams letting rushed care happen sometimes.
