Food sometimes too mashed up against some people's diets, risking choking or poor eating.

Some people were assessed by the speech and language therapists (SaLT) to receive food prepared as 'minced and moist' or 'soft and bite sized', however, their meals were served to a pureed texture.

Care records had mistakes like wrong health conditions listed, not caught by checks.

Audits and checks of people's care records had not identified inaccuracies... a person's record stated they had Parkinson's disease... neither person had a care plan in place for these conditions.

Staff training records had gaps that needed fixing with a new programme.

The registered manager identified gaps in training records and had arranged a training programme to ensure all staff had the relevant knowledge.
