No permanent manager in charge, so leadership feels unstable and oversight is weak.

At the time of the inspection, there was no registered manager in post.

Medication errors like gaps in records and unlabelled creams, plus safeguarding not always effective and lessons from incidents not shared well.

topical preparations were not well managed, and we found occasional shortfalls in medicines record keeping... lessons learned from safety incidents were not always shared and discussed with the staff team... The service did not always ensure safeguarding procedures in place were effective.

Some staff do not feel valued by managers or able to speak up confidentially without worry.

others did not feel valued or that they could speak confidentially... some staff told us they did not feel confident to raise concerns or whistle blow.
