during our observations in areas where people could not use call bells, we noted occasions when individuals were calling out or showing signs of distress without staff present to respond.

Safety risks like unsecured medicine cabinets and wrong cups increasing choking danger not always spotted or fixed before inspection.

Cabinets in people’s bathrooms containing medicinal creams and other items that may cause harm were not secure... the key was accessible; some people drinking from plastic cups with spouts, but not all had recommendations from a speech and language therapist.

Some staff do not always feel listened to or fully supported by leaders.
