"Staff feel supported by approachable managers with engaging activities, but medication errors and ineffective audits hold it back."
Not enough improvement had been made... and the provider was still in breach of regulation 12... Medicines which were prescribed to be given at specific times... were not always given at the correct times... Written guidance... was not always in place... for staff to follow when medicines were prescribed to be given "when required".
Staff were complimentary about the management team and told us they found the management team supportive and their opinions were welcomed and valued. One staff member told us, "They're good and approachable."
People told us they generally felt there was enough staff around... However, ... negative feedback about agency staff used at night. ... the home was using some agency staff to cover the gaps; these were mainly night shifts.
There was a comprehensive training and induction programme in place... However, the training matrix indicated several staff did not always have up to date/refresher training in place.
Systems and processes to ensure oversight of the service were not always effective. Audits completed had not always actioned the concerns we found on inspection. We identified a repeated breach and a new breach of regulations.
Per 2021 comprehensive inspection: People told us there were activities taking place. These included, bingo, general knowledge quizzes, games, sing-a-longs with wine, one to one outings, reminiscence sessions and crafts.
Staff make mistakes giving medication like wrong times, missing doses of pain relief or laxatives, and not applying creams or patches properly.
Medicines which were prescribed to be given at specific times or with specified time intervals between doses were not always given at the correct times... Written guidance... was not always in place... A system was in place to record the application of creams, but we saw that this system had not always been followed... Medicated patches were not rotated... The service did not use stool charts... One person was prescribed a laxative... however, this was not given each day.
Managers do not always spot problems through their audits and checks, so issues like medication errors keep happening.
Audits completed had not always actioned the concerns we found on inspection... The provider had not always ensured full oversight of the operations of the home, leading to the concerns identified on this inspection. This was a breach of regulation 17.
Several staff are behind on refresher training, so skills might not be fully up to date.
the training matrix indicated several staff did not always have up to date/refresher training in place. We recommend the provider ensures staff have completed refresher training in line with their own policy.
