People’s individual risk assessments had not been kept up to date and accurate to record changes to people’s risks and how these were being managed.

When we first visited the service, there was no staffing level assessment tool in use to identify the necessary level of staffing to meet people’s needs... there were not enough staff providing care.

The training matrix recorded 64% of staff members had completed their required training, and no records were made available to show new staff members had completed their induction training.

There was no culture of continuous improvement or effective management. The provider’s audit and oversight processes had not identified multiple issues or improved the quality of the service.

Peoples’ care was not being planned and delivered to meet the needs of the whole person, including the planning and provision of social and stimulating activities to meet the needs of the individual person.
