No permanent manager in post, so leadership is failing to spot key risks and issues.

At the time of our inspection there was not a registered manager in post... Systems to monitor the quality and safety of the service had failed to enable the management team to identify some of the concerns we found on inspection.

Staff not always investigating injuries properly or reporting possible abuse to authorities.

The provider had failed to identify some incidents needed further investigation... failed to notify the local authority's safeguarding team when concerns of the risk of possible abuse had been identified... We saw a cut on a person's arm for which a body map and incident form had not been completed.

Lots of care staff missing training on dementia, mental capacity, learning disabilities and other key areas.

Most care staff had not received training... such as the Mental Capacity Act, Dementia and other health conditions... Staff had not had training in this area [learning disabilities] in line with Government guidance.
