Medication safety has problems like unchecked storage temperatures and missing records for crushed meds or patch locations.

monitoring of temperatures of storage areas were not escalated when it was outside of the recommended range; this meant there was a risk medicines were not safe to give to people. For people who had their medicines crushed prior to administration this wasn’t always documented... For medicines patches there was not always a record of where this was applied

Some staff have overdue refresher training even though the manager is working to fix it.

some staff needed to complete refresher training however the registered manager was actively monitoring and managing this

Audits and checks missed key issues like medication records and consent forms.

whilst audits and checks were being completed, these did not identify and resolve the issues we found on inspection... People did not give consent to their care and those people who were unable to make their own decisions did not have documentation
