People were not always protected from the risk of harm as accidents and incidents were not comprehensively reviewed to ensure appropriate action was taken.
We received mixed responses regarding how well supported staff felt. Other staff members told us their relationship with the provider could be more difficult. They relayed examples of inappropriate comments made to them which they found disrespectful.
The majority of people we spoke with told us they felt staffing levels needed to increase. One person said, 'Staff have too much to do and have too many hats to wear. They need more help.'
The competency of 11 staff members had not been assessed in areas including moving and handling, providing oral health care, providing person centred care and supporting with oral health care.
The provider had failed to ensure robust oversight of the service and had not recognised the seriousness of concerns raised.
We observed activities could be repetitive and people who needed or preferred to spend time in their rooms had little opportunity for social interaction.

Incident records showed a number of safeguarding concerns which had not been reported using the correct channels... These concerns had not been identified as safeguarding concerns by the manager or provider.
