No permanent manager in post, so leadership oversight might be shaky.

At the time of our inspection there was not a registered manager in post.

Medication errors like missing stock and wrong timing risked overdoses.

Medicines were not always available to be administered... paracetamol containing medicines were administered to three people without leaving a four-hour gap. This placed people at risk of an overdose

Staffing is adequate now but high turnover recently means lots of new staff and uneven skills on shifts.

There had been a high turnover of staff in the months prior to the inspection and some staff were new to their roles. We raised some concerns about the skills mix of staff on some floors.
