Accident records sometimes miss details on why falls happen and how to stop them repeating, so learning from mistakes could be better.

The evidence of steps taken to prevent recurrence of falls were not always clear within the care documentation. Details logged... did not always include enough information about the root cause... We recommend the service considers training... in how to conduct comprehensive investigations.

Activities are regular with coordinators running fitness and social sessions, but no specialist programmes or partnerships to make the role more varied.

There were activities coordinators present on both floors who attempted to engage people and actively provide social stimulation... We observed two people engaged in one to one fitness sessions.
