Whilst on site we identified a significant incident which had not been reported to safeguarding or CQC and the lessons learnt document was not sufficiently robust.
Staff felt there was an improving picture with more consistent management support across the day and out of hours, more staff and regular staff meetings where staff felt they could give feedback.
