Risks like fire safety, choking and diabetes aren't always fully assessed, so staff can't be sure they're keeping people safe.

Risks relating to fire safety were not always safely managed... risks to people were not always fully assessed and documented to keep them safe. For example, where people had swallowing difficulties... no specific choking risk assessment in place... risks associated with the condition had not been sufficiently documented.

Managers' quality checks missed big problems with risks, training and records that inspectors found.

The provider had systems in place to monitor the quality and safety of the service, however they did not identify the issues we found during the inspection.

Training doesn't always meet national standards, especially restraint training which isn't certified properly.
