Safety risks from poor medication recording, wrong mattress settings, and out of date fire equipment put people at harm.

We observed controlled drugs were not always administered and recorded by staff in line with the service medicine policy... systems were not in place to robustly monitor mattress settings... fire evacuation chair which was out of date.

Gaps in staff training for dementia and other needs, plus lack of regular supervisions, mean skills aren't fully up to date.

Training records showed there were gaps in staff training linked to people’s needs... training matrix reflected some gaps in training. Staff reported a lack of regular supervision throughout the year.

Few activities beyond watching TV or sitting in the garden, so days feel routine with little variety or outings.

People reported they often spent their time either in the lounge or in their bedrooms, primarily watching television. Several people stated they would like to go out more than once a week.
