Risks to people were not well managed or mitigated and medicines were not managed safely. Records showed whilst some concerns of a safeguarding nature had been raised with the local authority as required, injuries that should have been reported to CQC had not.
We found the culture of the service was closed with a low staff morale. Staff did not feel they could report concerns openly to the provider and there was a clear divide between some staff and the provider.
Records showed the recruitment process was not safe and the provider had failed to follow and implement their own recruitment policy. Most people and relatives told us overall they felt the service needed more staff, as there were not enough staff readily available to support people or engage in activities.
The provider had a staff training matrix in place, but this was not accurate or up to date and had failed to include 5 new staff members. Some staff had not had training in some of the mandatory subjects such as moving and handling which placed people at risk of harm.
The governance systems in place had failed to identify and rectify the widespread and significant concerns found at this assessment. Staff did not feel cared for or supported by any of the management team. Feedback included, “The provider shouts at staff if we aren’t doing what they have told us to, they bully the staff and are not nice”
Despite having an activity schedule, the home did not have regular opportunities, so people were left for long periods of time with no meaningful activity or encouragement to move or exercise. Multiple people and relatives we spoke to felt the service needed more staff as there were not meaningful, engaging activities in place.
